Discharge Planning
Planning for your discharge from the hospital really begins the day you are admitted.  A team that includes your physician, the Case Management department and your nurses work to plan for your discharge.  If you need additional medical follow-up at home, your physician may request home health, medical equipment, physical therapy, respiratory care or other services.  These and other community services to assist you are arranged as part of the discharge planning.
