Job Description
Primary Function Plans, evaluates, provides and documents physical therapy to patients in the home setting. Conduct physical therapy assessments, develops plans of care, provides education and discharge planning to patients and their families. Participates in Performance Improvement programs. Contributes to overall patient satisfaction through interaction with patients, families, the interdisciplinary team, and physicians. Performs other duties as assigned. All duties and standards within this job description will be performed according to established policies, procedures and guidelines outlined in the department and hospital policy and procedure manuals.
Educational Requirements Graduation from an accredited Physical Therapy program
Certificates, Licenses, and Registration Current FL PT license; current driver’s license; current automobile insurance
Skills and Experience Two (2) years of physical therapy experience is preferred. Must have good oral and written communication skills. Must be able to communicate with patients, patient families, physicians, and other healthcare providers effectively and professionally. Must be willing to accept responsibility, and be able to maintain confidentiality of information.
